Important Message:
Many of you in charge of Concert Festival did not include W-9's for the judges with your check requests and are not signing the voucher forms (you may have assumed that if it is less than $600.00 this is not required, however many judges pull double duty and you have no way of knowing that. Please submit a W-9 form each time you request payment for services). W-9 forms can be found on the FORMS PAGE. Please note: Sharon will be returning all incomplete paperwork from this point forward - there is no way to keep up with each of you after the fact.

Please do not use the purple "example" rows to type your students' information in the excel file. This will result in your numbers being incorrect. You can begin to type your information in row 13. If you have already completed the form, please check to make sure the numbers are accurate. (Thank you to John Kvasnicka for catching the problem!)

Postmark deadline for S&E registration has been extended to March 10.
NO registrations will be accepted after March 25 and registrations sent later than March 10 will be subject to a late fee.
